Job Description

POSITION SUMMARY
The Care Coordinator plays a critical role in supporting Medicaid members through short-term care coordination focused on transitions of care, facilitating access to needed services, and collaborating with healthcare providers and community partners. This position is designed to ensure that members receive timely support following hospitalizations or significant health events before being connected with ongoing care coordination resources when appropriate. The Care Coordinator will also support facilitation and participation in Creative Solutions meetings and cross-system staffing with NHP, providers, and other stakeholders.

Primary Responsibilities
  1. Support members during care transitions (e.g., hospital discharge, residential treatment step-down) to ensure continuity of care and reduce avoidable readmissions.
  2. Conduct initial outreach and needs assessments to build rapport and identify immediate physical, behavioral, and social needs.
  3. Assist members in navigating the healthcare system, including scheduling follow-up appointments, providing referrals, and coordinating timely services.
  4. Facilitate or participate in care coordination staffing and creative solutions meetings with internal teams and external partners to address complex needs.
  5. Coordinate with NHP’s clinical teams to hand off members for ongoing care coordination when extended support is needed.
  6. Provide brief education to members and families on care plans, medication follow-up, preventive care, and available community resources.
  7. Maintain accurate and timely documentation in accordance with privacy and compliance standards.
  8. Build collaborative relationships with providers, community-based organizations, and social service agencies.
  9. Travel throughout NHP/Region 2 to support member engagement, provider meetings, and community collaboration.

KNOWLEDGE/SKILLS/ABILITIES

Required Education and Experience
  • High School Diploma/GED or 4+ years of community outreach or care coordination experience
  • Minimum 3 years of experience working with diverse populations, ideally in healthcare, behavioral health, or social services
  • Intermediate computer skills (Microsoft Office, web-based applications)
  • Reside in Colorado and have access to reliable high-speed internet (if remote/hybrid)
  • Access to reliable transportation for travel up to 25% of the time 
  • Valid Colorado driver’s license and required insurance
Preferred Qualifications
  • Prior experience in Medicaid or state-sponsored health programs
  • Knowledge of behavioral health systems and/or physical health care delivery
  • Familiarity with community-based resources across NHP’s region
  • Experience with creative problem solving for complex member needs
  • Bilingual in English and Spanish
  • Familiarity with the child welfare or foster care system
Knowledge, Skills and Abilities
  • Understanding of medical and behavioral health terminology
  • Familiarity with healthcare regulations, especially HIPAA and Medicaid guidelines
  • Strong interpersonal communication skills with the ability to engage members empathetically
  • Ability to manage multiple priorities in a fast-paced environment
  • Confidence navigating cross-system communication (e.g., hospitals, behavioral health providers, justice system, child welfare)
  • Ability to work independently and within a team structure
  • Strong organizational skills and attention to detail
WORKING CONDITIONS
Work is primarily performed in a standard office environment, in Greeley, CO; however, a hybrid work model may be offered, blending in-office and remote work opportunities based on organizational needs and job requirements. Some meetings will occur at locations outside of the usual work location, including the use of conference and electronic meeting platforms (e.g., Zoom, Teams). All positions require travel within the counties Northeast Health Partners serves.
